Involving skin and mucous membranes
IV 20mg/kg 8 hourly for 14 days

Disseminated disease or CNS involvement
IV 20 mg/kg for 21 days

HSV infection in Immunocompetent host
Oral
1 - 23 months
100 mg 5 times per day for 5 days (longer if healing incomplete)

>=2 years
200 mg 5 times per day for 5 days (longer if healing incomplete)

HSV infection in immunocompetent host with severe mucocutaneous disease +/- CNS involvement
(if CNS involvement present duration is extended for 21 days or longer until CSF is free of HSV)
IV infusion

20 mg/kg 8 hourly for 14 days

3months - 11 years
250mg/m2 8 hourly for 5 days

>=12 years - 5 mg/kg 8 hourly for 5 days

HSV infection in immunocompromised host
